Clinical trial finds improvement in cognitive function but no difference between group's after physical, mental activity in older adults
Apr. 1, 2013 ? A randomized controlled trial finds that 12 weeks of physical plus mental activity in inactive older adults with cognitive complaints was associated with significant improvement in cognitive function but there was no difference between intervention and control groups, according to a report published Online First by JAMA Internal Medicine, a JAMA Network publication.
An epidemic of dementia worldwide is anticipated during the next 40 years because of longer life expectancies and demographic changes. Behavioral interventions are a potential strategy to prevent or delay dementia in asymptomatic individuals, but few randomized controlled trials have studied the effects of physical and mental activity together, according to the study background.
"We found that cognitive scores improved significantly over the course of 12 weeks, but there were no significant differences between the intervention and active control groups. These results may suggest that in this study population, the amount of activity is more important than the type of activity, because all groups participated in both mental activity and exercise for [60 minutes/per day, three days/per week] for 12 weeks. Alternatively, the cognitive improvements observed may be due to practice effects," the authors note.
The study by Deborah E. Barnes, Ph.D., M.P.H., of the University of California, San Francisco, and colleagues included 126 inactive, community-dwelling older adults with cognitive complaints. All the individuals engaged in home-based mental activity (1 hour/per day, 3 days/per week) plus class-based physical activity (1 hour/per day, 3 days/per week) for 12 weeks and were assigned to either mental activity intervention (MA-I, intensive computer work); or mental activity control (MA-C, educational DVDs) plus exercise intervention (EX-1, aerobic) or exercise control (EX-C, stretching and toning). The study design meant there were four groups: MA-I/EX-I, MA-I/EX-C, MA-C/EX-1 and MA-C/EX-C.
Global cognitive scores improved significantly over time but did not differ between groups in the comparison between MA-I and MA-C (ignoring exercise), the comparison between EX-I and EX-C (ignoring mental activity), or across all four randomization groups, according to the study results.
"The prevalence of cognitive impairment and dementia are projected to rise dramatically during the next 40 years, and strategies for maintaining cognitive function with age are critically needed. Physical or mental activity alone result in small, domain-specific improvements in cognitive function in older adults; combined interventions may have more global effects," the study concludes.
Commentary: Can Participation in Mental, Physical Activity Protect Cognition in Old Age?
In an invited commentary, Nicola T. Lautenschlager, M.D., of the University of Melbourne, Australia, and Kay L. Cox, Ph.D., of the University of Western Australia, Perth, write: "Barnes and colleagues should be commended for reporting the results of the MAX trial in the international literature. Although the overall trial results were negative, there is a positive message and several points that can be learned from these findings."
"First, the authors have demonstrated that stimulating activity, either mental activity or exercise, can improve cognition in only 12 weeks, even in older adults with cognitive complaints. Second, short-term interventions in well-controlled RCTs [randomized controlled trials] may not be long enough for the intervention to be effective," they continued.

German opposition parties warn each other: don't team up with Merkel

BERLIN (Reuters) - Germany's two main opposition parties traded warnings on Sunday against joining forces with Chancellor Angela Merkel's conservatives after September's election if they fail to win their own left-of-center majority.
The leaders of the Social Democrats (SPD) and Greens party issued unusually shrill messages to each others' supporters about the risk their votes might end up going to a party that could join forces in a coalition with Merkel.
The SPD and Greens want to form a center-left government after September's election but opinion polls show they will fall short of the needed margin. Surveys show Merkel's best chances of serving a third term could be to lure either the SPD or the Greens into a coalition with her Christian Democrats (CDU).
SPD chairman Sigmar Gabriel, alarmed about flirtations between the Greens and CDU, told the Welt am Sonntag newspaper that Greens voters should be aware that the pro-environmental party could end in bed with the CDU if the SPD and Greens failed to achieve a majority on September 22.
"That can't be ruled out and Greens voters should know about that," said Gabriel. The Greens, he said, had turned into Germany's "new liberal party" as they were chasing voters who had earlier backed the CDU and pro-business Free Democrats (FDP).
"There is a lot of overlap between the CDU and the Greens now," Gabriel said, trying to deter hard-core Greens voters who have traditionally viewed the conservatives as the party's arch political enemy.
'HOT AIR MERCHANT'
Greens co-chairman Cem Oezdemir, one of the party's most eloquent proponents of an opening to the CDU, quickly shot back.
"Gabriel is a hot air merchant," Oezdemir told Die Welt newspaper. "He knows that we want to defeat the center-right government together with the SPD. But it's not enough to rely only on SPD-Greens voters. If we were to do that, the SPD would quickly end up turning to Merkel for another 'grand coalition'."
Opinion polls show neither Merkel's ruling center-right coalition nor the center-left opposition command enough support to win a majority to lead Europe's biggest economy.
An Emnid poll in Bild am Sonntag showed Merkel's conservatives at 39 percent and their Free Democrat allies at five percent for a combined 44 percent. The SPD was at 26 percent and the Greens at 15 percent for a total of 41 percent.
The SPD and Greens governed together in a coalition from 1998 to 2005. After that the SPD joined forces with Merkel as junior partners in an awkward "grand coalition" until 2009.
The Greens' support in polls has doubled in recent years to levels around 15 percent thanks in part to fears about nuclear power in the wake of the Fukushima nuclear disaster, but also their growing appeal to conservative and rural voters.
The Greens, once famous for their unpredictable and self-destructive party congress battles, have become a serious and united party eager to return to power.
At the state level, the Greens ruled in a harmonious coalition with the CDU in Hamburg for three years until 2011, earning them national respectability as a fiscally responsible party. They have also ruled the state of Baden-Wuerttemberg with the SPD as their junior coalition partners since 2012.

Google says to shut down YouTube in early April Fools' gag
SAN FRANCISCO (Reuters) - Google Inc, getting a headstart on the annual tradition of April Fools' pranks, released a YouTube clip on Sunday declaring that the world's most popular video website will shut down at the stroke of midnight.
The three-minute video intended as a gag - a montage of clips and cameos from viral video stars like David Devore from "David after the dentist" - describes how the website will wind down as some 30,000 technicians begin to trawl through 150,000 clips, to select the world's best video.
The winner gets a $500 stipend, a clip-on MP3 player - and becomes the sole video to be featured on YouTube when the website relaunches in 2023.
"Gangnam Style has the same chance of winning as a video with 40 views of a man feeding bread to a duck," YouTube CEO Salar Kamangar pronounced, referring to the viral sensation from Korean pop artist PSY that's now the most-viewed video on the site.
Google's video also features intense discussions between judges, who hotly debate the merits of everything from Citizen Kane to "epic skateboard fail". While clearly tongue-in-cheek, several YouTube viewers appeared stricken or dumbfounded, while others expressed sadness and regret in attached comments.

Planning to snag that maxed out HTC One in the US? AT&T is where you'll have to go, then. The 64GB variant of the device is an AT&T exclusive, as revealed by a recently uploaded video to the service provider's YouTube channel. The 32GB One will also be on offer, though there's no word on exactly how much either will be priced at or an exact arrival date -- April is the most specific we've heard from HTC. Verizon is also expected to carry the device, of course, but AT&T will certainly be "the one" for folks needing that doubled storage space on a two-year agreement.
